hi this is dr. philip young from seattle bellevuewashington. in this video i’ll be presenting otoplastyand its revision surgery. here i am in the operating room ready to takeon an otoplasty revisional challenge here is a before and after for a person whohad multiple otoplasties with her ears pinned too close to her head prior to coming to us. her after images show a markedly improvedposition that she wanted after we reversed her excessively pinned earshere i am assessing the revision i’m about to beginthis is a young asian female with a cup lop ear deformity who i corrected and also pinnedback for a more pleasing look.

the cup lop ear presents a more significantchallenge to the otoplasty surgeon here is a before and after correction of akeloid in the ear this procedure was done without sacrificingtoo much of the surrounding normal ear structures. the patient was extremely happy with her resultshere is an asian 20 something female with a classic excessively protruding earher after image shows a great aesthetic result without being pinned back too much. it was just what she wantedhere is the angled view of the cup ear lop ear deformity we showed earliershe was excited to finally be able to wear her hair back.

you can have that same confidence after otoplastybecause of my work, i was contacted by a well known author of plastic surgery textbooksto write a chapter on revision otoplasty. you can find my chapter in advanced cosmeticotoplasty published by springer in 2013 here are more otoplasty images. i noticed that when the ear was pinned backthe middle part still stuck out. after a discussion with the patient, we restructuredand molded the middle part so that it laid flatter than the outer part of the ear fora better look in the end this is an angled view of the middle partafter reconstruction showing a smoother less sharp and less protruding middle portion ofthe ear.
